About us
We're Holifya , an Italian healthtech building the digital clinic for metabolic health and we are committed to becoming market leaders. Real doctors, real clinical protocols, GLP-1 therapies — delivered through software that makes the whole journey simpler for the patient and the medical team, and where data is king.
We're already caring for 1,500+ patients with 15+ clinical specialists , and we're growing super fast (8x by beginning of year). So far our platform has been built by freelancers, and we've hit the ceiling of what that setup can do. Now we want to bring engineering **in-house** — and you'd be the first dedicated engineer to do it.
This is the moment where the product stops being a bottleneck and starts being our edge. We're looking for the person who makes that happen.
What you'll do
- Own features end-to-end — from a problem framed together with our Product Manager and Medical Lead all the way to production: frontend, backend, and everything in between.
- Strengthen our backend and data foundations — the part that lets us collect clinical data cleanly and build real things on top of it (this is where most of the work lives), integrating third party APIs for more data collection too.
- Ship every week. Small, safe releases over big rewrites. Validate, measure, iterate.
- Decide what we build, buy, or migrate — you'll have a real say in the architecture and the tools.
- Integrate the services the product runs on — payments, booking, chat, email, and patient-data sources over time.
- Use AI daily to multiply your output , and help us build AI _into_ the product (protocol assistants, adherence algorithms) — for clinical impact, not marketing.
- Grow into a Tech Lead role as you prove yourself, and help shape the engineering team we build next.

What we're looking for
Non-negotiables:
- Milan-based , able to work in person 2–3 days/week — this is a hard requirement.
- Professional working proficiency in Italian and English.
- 5+ years building and shipping real products (7+ preferred ).
- Full-stack with genuine backend strength — you've designed schemas, APIs and data models, not just UIs. (Frontend-only profiles aren't the right fit for this role.)
- Fluent with a modern stack: TypeScript, Node, React, PostgreSQL, Supabase or a similar BaaS.
- Ownership mindset — you frame problems, propose solutions, and ship without hand-holding.
- Product- and business-minded — you care that what you build moves the company, not just that it compiles.
Nice to have:
- Exposure to healthtech, telemedicine or regulated/sensitive-data environments (GDPR).
- Experience shipping AI/ML features in production (not just prototypes).
- An early-stage startup or high-intensity engineering background.
You're the ideal person if…
You think in systems, not screens. You're comfortable trading perfection for speed at this stage, because you know an early-stage product wins by validating fast. You like owning a problem from the first conversation to the deploy, and you want the responsibility — and the growth path — that comes with being the first engineer in the room.

You'll be our first dedicated engineer, working shoulder-to-shoulder with our Founders, Tech Advisor, Product Manager, Tech team freelances. You're not inheriting a faceless backlog — you're shaping how we build, with a path to grow the team around you.
Why hybrid and not remote?
We're early stage, and being in the same room a few days a week is how we move fast and learn together. We're flexible on the exact days, not on being in Milan.
