Description

We are seeking two (2) experiencedCase Coordinator to join our Perth team, responsible for supporting clients in Perth, Toodyay and the surrounding areas . This role is ideal for someone who has experience coordinating Government-funded Home Care Packages (HCP) and who is passionate about delivering client-focused care that supports independence, dignity, and wellbeing.

As a Case Coordinator, you will work primarily from our Perth office , with work conducted mainly over the phone. Some travel to Toodyay and the Wheatbelt will be required, and a business car will be available when needed.

Your role will involve liaising with hospitals for new referrals, sourcing external providers, and supporting the growth of our services through professional networking.

Key Responsibilities

  • Coordinate and manage client Home Care Packages (HCP) .
  • Conduct assessments, develop, and review individualised care plans.
  • Liaise with hospitals and healthcare providers to facilitate new client referrals.
  • Source external referrals and build professional relationships to increase service growth.
  • Provide support and advice to clients and families regarding services and funding.
  • Ensure compliance with Aged Care legislation and organisational policies.
  • Maintain accurate client documentation and records.

Qualifications

  • Previous experience in coordinating Home Care Packages.
  • Solid understanding of Aged Care legislation and compliance requirements.
  • Strong communication and interpersonal skills.
  • Excellent organisational and time management skills.
  • Confident in managing client needs via phone and liaising with multiple stakeholders.
  • National Police Clearance (current).
  • Current First Aid and CPR certification (or willingness to obtain).
